Terms and Conditions for Loop Medical Center Effective Date: May 5, 2025
1. Agreement to Terms
Welcome to Loop Medical Center. These Terms and Conditions (“Terms”) constitute a legally binding agreement made between you, whether personally or on behalf of an entity (“you” or “patient”), and Professional Anesthesiology Interventional Network, Inc., doing business as Loop Medical Center (“Loop Medical Center,” “we,” “us,” or “our”), concerning your access to and use of the https://loopmedicalcenter.com/ website (the “Site”) as well as any related non-clinical services provided through the Site. Our clinical services, provided at our location at 1921 S Michigan Ave, Chicago, IL 60616, are governed by these Terms where applicable, as well as by separate patient agreements and applicable healthcare regulations.
You agree that by accessing the Site, you have read, understood, and agree to be bound by all of these Terms. IF YOU DO NOT AGREE WITH ALL OF THESE TERMS, THEN YOU ARE EXPRESSLY PROHIBITED FROM USING THE SITE AND YOU MUST DISCONTINUE USE IMMEDIATELY.

All appointments are subject to availability and confirmation by our staff. The provision of medical services is subject to a formal patient-physician relationship and applicable clinical protocols.
7. Payment Terms and Financial Responsibility
You are responsible for understanding your insurance coverage and for all charges associated with the clinical services you receive. This includes co-payments, deductibles, co-insurance, and charges for services not covered by your insurance plan. Payment is typically expected at the time of service unless other arrangements acceptable to Loop Medical Center are made in advance. We may utilize third-party billing services. 8. Cancellation and Rescheduling Policy
Please provide at least 24 hours’ notice if you need to cancel or reschedule your appointment. This allows us to offer the time slot to other patients needing care. Failure to provide 24 hours’ notice or missing your appointment without notification (“no-show”) may result in a cancellation fee of $100 (one hundred dollars). Repeated missed appointments may lead to discharge from the practice. Please call us at 312-414-1088 to make changes to your appointment.
9. Patient Responsibilities
As a patient, you agree to:
- Provide complete and accurate medical history and current health status, including all medications, allergies, and previous treatments.
- Actively participate in your treatment plan and follow provider recommendations.
- Attend scheduled appointments or provide adequate notice if unable to do so.
- Treat our staff and other patients with respect.
Fulfill your financial obligations for services rendered. We reserve the right to terminate the physician-patient relationship in accordance with applicable ethical guidelines and regulations.

13. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of Illinois, without regard to its conflict of law provisions.
14. Dispute Resolution
Any dispute arising from or relating to these Terms or your use of the Site (not clinical services, which may be subject to separate agreements or specific legal procedures) shall be resolved exclusively in the state or federal courts located in Cook County, Illinois.
15. Modifications to Terms

17. Miscellaneous
